Q. How do I get a copy of my records to another provider? 

A.The patient can contact our correspondence department with the other providers name, address, phone number and fax number. The date of the upcoming appointment is also helpful.

Q. Is there a charge for copies of my medical record? 

A. If we are sending the records to another provider or if the patient is going to hand carry them to another provider, there is no charge. Records for personal use, legal purposes, or for supplemental insurance claims are subject to copying fee.

Q. How do I assure that my spouse/adult child/someone else I designate has access to my medical records?

A.The patient needs to complete and sign a Designation of Personal Representative form. These can be obtained at the clinic or on our website under the Contact/Forms tab.
