Enroll in the New Patient Portal 

You’ve asked, we’ve listened.  We are pleased to announce the new Yankton Medical Clinic, P.C. patient portal has been launched.

Our new patient portal offers better communication between you and your physician through convenient, secure online access from your web-enabled device- whether in the comfort and privacy of your home, office or on the go, with no app download necessary.

Sending messages to your provider can be done at any time through the patient portal. 

If you are experiencing a medical emergency, call 911 or your local emergency number for immediate assistance.

Through the patient portal you’ll be able to:

  • Request appointments
  • View medications
  • Request a medication renewal
  • Retrieve immunization history
  • Review allergies & health history
  • Send your provider a message
  • View test reports*
  • View radiology reports*
  • View visit summaries
  • Update health information
  • Complete forms
  • Access your medical record and those of your dependents
  • Update notification preferences for:
    • Appointment reminders
    • New appointments
    • New medications
    • New messages

*Reports are available once the physician signs the report and sends to the patient portal. Visit with your provider about options to receive testing reports.

Enrollment

Enrollment will be required even if you had an account with the previous portal.  It’s quick and easy.  If you are an active portal user (logged in in the last 90 days), you will automatically receive an email from patientservices@medfusion.net to enroll beginning Oct. 18.  If you don’t see an email, check your spam or junk folders.

  • Click the link in the invitation email
  • Create a username and password
  • Access your account

If you haven’t accessed your portal account in a while or don’t have one now that’s ok, just verify your email with anyone on our staff at the clinic and we will send you an invitation email.

Access to your patient portal account through Intellichart will discontinue as of October 15, 2021.  After this date messages and requests will not be monitored.  All current health information and messaging will be available through the new patient portal through Medfusion. Be sure to update any saved bookmarked links to the portal.
